<p><b>OMB Control Number Info</b></p>

<p>The Office of Management and Budget has approved our collection of this information under Control Number 1660-0002 until July 31, 2025. The application we send will have this number and date at the upper right-hand corner. </p>

<p>If the number is not current, FEMA may not conduct the interview and the applicant does not have to respond to our questions.</p>

<p>In the script and privacy act statement, FEMA complies with the Paperwork Reduction Act of 1995 by telling the caller the following: </p>

<ul><li>The reasons why we collect the information.</li>
<li>The way we use the information.</li>
<li>The estimate of time to complete the interview.</li>
<li>That we require the information for them to get assistance.</li></ul>

<p>If the caller wants to comment on the time estimate or suggest ways to reduce it for the interview, they may write to: Information Collections Management, Paperwork Reduction Project 3067-0009, FEMA, 500 C Street, SW, Washington, DC 20472. </p>

<p><b>CITIZENSHIP DEFINITIONS</b></p>

<p><b>U.S. Citizen</b></p>

<ul><li>A person born in one of the 50 states, the District of Columbia, Puerto Rico, Guam, the U.S. Virgin Islands, or the Northern Mariana Islands.</li>
<li>A person born outside of the U.S. to at least one U.S. parent.</li> <li>A naturalized citizen.</li></ul>
<p><b>Non-citizen National</b></p>
<ul><li>A person born in an outlying possession of the U.S. (like American Samoa or Swain's Island) on or after the date the U.S. acquired the possession, or a person whose parents are U.S. non-citizen nationals. All U.S. citizens are U.S. nationals, but not every U.S. national is a U.S. citizen.</li></ul>
<p><b>Qualified Non-citizen</b></p>
<ul><li>A legal permanent resident ("green card" holder).</li>
<li>An asylee, refugee, or a non-citizen whose deportation is being withheld.</li>
<li>A non-citizen paroled into the U.S. for at least one year.</li> <li>A non-citizen granted conditional entry (per law in effect prior to April 1, 1980).</li>
<li>A Cuban-Haitian entrant.</li> <li>A non-citizen in the U.S. who (or whose child or parent) was abused, subject to battery or extreme cruelty by a spouse, other family or household member, or were a victim of a severe form of human trafficking.</li></ul>

<p><b>OWNERSHIP DEFINITIONS</b></p>

<p><b>Three Basic Types of Ownership:</b></p>

<ul><li><b>Legal Ownership:</b> When a person holds the deed and title to the home or property.</li>
<li><b>Beneficial Ownership:</b> When a person has lifetime occupancy rights, but someone else holds the legal title. The owner-of-record must provide a notarized affidavit to certify the lifetime occupancy rights of the applicant. They must also provide proof of their legal ownership of the property.</li>
<li><b>Rent-free Ownership:</b> When a person does not hold legal title to the home, and pays no rent, but is responsible to pay the taxes and maintenance costs for the home. A copy of tax payment or maintenance receipts may be used as proof of status.</li></ul>

<p><b>LATE APPLICATIONS</b></p>

<p>Late applications may be completed for up to 60 days from the standard or extended filing date. Applicants must submit all requested documents to support the reasons why they were not able to apply before the deadline.</p>
