<b>Social Service Organization Letter</b><br/><br/>

<p><i>This policy change is in effect for disasters declared on or after August 13, 2021.</i></p>

<p>To be considered for FEMA assistance, you may provide a letter or written statement for one of the following from a social service organization (such as the Center for Independent Living, Meals on Wheels, etc.):</p>

<ul><li>A document received before the disaster that proves they provided services to the applicant or co-applicant at the damaged dwelling address. Or</li>

<li>A letter or other written statement prepared after the disaster that verifies the applicant or co-applicant lived at the damaged dwelling address at the time of the disaster.</li></ul>

<p>The letter or written statement must include the name and telephone number of the person or organization providing the proof.</p>

<p>You may use this document to verify occupancy.</p>
