<b>Upload History</b><br><br>
This screen displays a history of every document you upload. Documents sent to FEMA by fax or through postal mail will not display in the <b>Upload History</b>.<br><br>Faxes, postal mail and uploaded documents sent to FEMA will display on the Correspondence screen in the “Received from You” frame.<br><br>If you have not uploaded any documents to FEMA through your account a temporary message will display on this screen stating that no files have been uploaded.<br><br>

The data displayed for you with each uploaded document includes the following columns :<br><br>
<b>Date/Time</b> records the actual date and time of day you uploaded your documents.<br><br>
<b>Assistance Type</b> records the Assistance Type you selected when you performed the upload.<br><br>
<b>Document Type</b> records the Document Type you selected when you performed the upload.<br><br>
<b>File name</b> records the actual File Name of each file you selected and successfully uploaded.<br><br>
<b>Status</b> records the current status of each uploaded document:<br><br>
<ul><li>Pending - your document has been uploaded and sent for a virus scan. Once the scan is complete, and the file is categorized, the status will change to View Document. You can then view the document on the <b>Correspondence Page</b>. </li>

<li>View Document – you can now view your document on the <b>Correspondence Page</b>.</li>

<li>Rejected – your file has either not passed the virus scan or was corrupted. Please try uploading the file again.

When sending documentation to FEMA, please be sure to include your full name, address, phone number, disaster number and registration ID on each page. The registration ID can be found in the top right corner of the screen. <br><br>
The address, phone and fax numbers for FEMA are located in the <b>Contact Us</b> at the top right corner of each page. You may need to use the scroll bar on the right side of the screen to view this information.<br><br>
