<b>Rental Recertification Process Information</b></p>

<p>If you are in need of additional rental assistance from FEMA and would like to apply, you will need to send the documentation listed below. The information applies to both owners and renters. (You may need to use the scroll bar to view all information).</p>

<p>When sending documentation to FEMA, please be sure to include your full name, address, phone number, disaster number and Registration ID on each page of your correspondence. Your Registration ID number can be found in the top right corner of the screen.</p>

<b>All applicants (<i>owners and renters</i>) will need to provide the following if in need of additional rental assistance:</b></p>

<ol>
<p><li><b>A completed Declaration of Continuing Need for Temporary Rental Assistance (from here on in referred to as &#34;Declaration&#34;).</b></p>
<p>This Declaration would have been mailed to you shortly after you received Rental Assistance.</p>
<p>The Declaration requires information related to your current housing status and your pre-disaster housing status and the costs related to each.<p/>

<dt>Section A.</dt>
<dd><p>Complete the Applicant Information at the top of the Declaration.
This section requires you to complete the blanks with your:</p>

<ul><p>
<li>Full Name</li>
<li>Current Address</li>
<li>Phone Number</li>
<li>FEMA Application Number</li>
<li>Disaster Number</li>
</p></ul>

<dl>
<dt>Section B.</dt>
<dd><p>Read <b>all</b> of the Statements in this section to be certain they apply to
your situation.</p>
</dd>

<dl>
<dt>Section C.</dt>
<dd><p>Complete all the blanks which apply to the costs of your pre-disaster housing and your current housing costs. Please note there is a section of the Declaration which is specifically for homeowners and one specifically for renters. (If you "lease to own" complete the renter section.)</p>

<dt>Section D.</dt>
<dd><p>Complete the blanks to declare your pre-disaster income and your income after the disaster (post disaster).</p><p>
<b>You MUST attach a copy of each of the following income documents that apply:</b></p>

<ul><p>
<li>Recent pay statements</li>
<li>Social Security income statements</li>
<li>Unemployment benefit statements</li>
<li>Retirement benefit statements</li>
<li>Any other documents reflecting the monthly income for all members of your current household who are 18
years of age and older</li>
</p></ul>

<li><p><b>A copy of your current LEASE.</b> If you have already sent your current lease to FEMA, you do not need to send it again. You <b>MUST</b> have a current lease agreement on file with FEMA to be considered for additional rental assistance.</li></p>
<li><p><b>Copies of rent receipts / checks:</b> You <b>MUST</b> submit a copy of all rent receipts
and/or cancelled checks showing you paid rent with the temporary rental assistance that FEMA previously gave you. If a rent receipt has already been submitted (and received) with a prior rental assistance request, you do not need to send a duplicate. However, rental receipts/checks must be submitted for all periods of rental assistance for which FEMA has assisted with your monthly rental charges.</li></p>
<li><p><b>Signature:</b> You <b>MUST</b> sign the Declaration. The Declaration <b><i>will not</i></b> be processed without your signature.</li>

<p>You must complete the <b>Declaration of Continuing Need for Temporary Rental Assistance</b> in full and provide any updates of required information (such as rent receipts/ cancelled checks) each time you request additional rental assistance.</p><p>

<p>If you use a video relay service, captioned phone, or other communication service, give FEMA the number for that service.</p>
<p>Please have your Registration ID number and Social Security Number available when calling.</p>
