The Social Security Administration is allowed to collect the information on this form under Section 205 of the Social Security Act. We need this information to quickly identify who you are and provide the information you requested. Giving us this information is voluntary. However, without it, we may not be able to give you the information that you want and make the requested change to your record. <br><br>
Although the information you furnish on the form is rarely used for any other purpose than stated, there is a possibility that information may be disclosed to another person or to another governmental agency to comply with Federal laws authorizing or requiring the release of information from Social Security records. For example, we may provide information in response to a request on your behalf from a Congressional office or the Office of the President.<br><br>
We may also use the information you give us when we match records by computer. Matching programs compare our records with those of other Federal, State or local government agencies. Many agencies may use matching programs to find or prove that a person qualifies for benefits paid by the Federal government. The law allows us to do this even if you do not agree to it.<br><br>
