To be considered for medical personal property expenses you must submit receipts/estimates showing the replacement/repair cost of the item damaged by the disaster and written verification from a physician stating your condition requires this item. Please include the name, address, and phone number of the estimate/receipt provider(s).<br>

If you have insurance that covers your medical personal property losses you must also submit either the written denial from your insurance carrier or the explanation-of-benefits statement for the amount(s) which they have covered.<br>

If your losses are NOT covered by insurance, please sign and date the Statement of Insurance form at the bottom of your letter, return it with the provider information and requested receipts.
