<b>Applicant Information:</b><br>
This page displays the personal information that is currently displayed in your file.</p>

<p><b>Damaged Address:</b><br>
This is the address of the property that was damaged or where the damage occurred. If the address shown is incorrect, send a letter to FEMA at the address under <b>Contact Us</b> at the top right corner of each page, along with proof of occupancy at the corrected address. (Example: Utility bill in your name for the correct damaged property address. Must be dated within three months prior to the incident.)</p>

<p><b>Mailing Address:</b><br>
This is the current mailing address on file where all messages sent by FEMA will be addressed. If this address is incorrect or no longer current, click <b>Edit</b> under <b>Mailing Address</b> and enter the correct address. Or send FEMA a letter to request a mailing address update at the address shown under <b>Contact Us</b> at the top right corner of each page.</p>

<p><b>Electronic Funds Transfer:</b><br>
Funds would only be transferred into your account <b>IF</b> you were found to be eligible for FEMA assistance. The name on this application must be the same as on the bank/savings account you identify. Do Not enter anyone else's account information. This service is not available for Business Only applicants.</p>

<p><b>Contact Information:</b><br>
This lists all the phone numbers currently displayed in your file. It includes your current phone number, the phone number at your damaged address, and an alternate slot for your cell number or alternate contact number.</p>

<p>If a phone number is incorrect or not current, click <b>Edit</b> next to that number and enter the correct number. Or send FEMA a letter to request an update for any of your phone numbers at the address shown under <b>Contact Us</b> at the top right corner of each page.</p>

<p><b>Insurance Information:</b><br>
This shows your insurance information, if you provided it to FEMA. It includes the name of your insurance company, the type of insurance coverage, your policy number, and the insurance company's phone number.</p>

<p>To add insurance policy information, click <b>Add</b>. To edit your existing insurance company information, click <b>Edit</b>.</p>

<p><b>Email:</b><br>
You must have an email address to create an account and get notifications from FEMA.</p>
