<p><strong>Will and Death Certificate</strong></p> <p>Please make sure your documents include:</p> <ul> <li>Your name or your co-applicant's name.</li> <li>Your home address (the address you used for your FEMA application).</li> <li>Proof that they were current/in effect at the time of the disaster, and&nbsp;</li> <li>Were dated prior to the disaster.</li> </ul> <p><strong>Affidavit of Heirship and Death Certificate</strong></p> <p>Please make sure your documents include:</p> <ul> <li>The deceased owner's name and date of death (before the disaster).</li> <li>The home address listed on your application.</li> <li>Evidence of your heirship or your co-applicant's heirship at the time of the disaster.</li> <li>The Affidavit of Heirship must comply with state or territorial laws and may be dated after the disaster if it shows the document was effective at the time of the disaster.</li> </ul> <p><strong>Receipt for Repairs</strong></p> <p>Please make sure your document includes:</p> <ul> <li>Your name or your co-applicant's name.</li> <li>Your home address listed on your application.</li> <li>Information that you or your co-applicant were responsible for major repairs or improvements to the damaged home.</li> <li>Proof that the repairs or improvements were within five years before the disaster.</li> <li>The name, address and contact information for the contractor.</li> </ul>
