Not Approved - A completed and signed Declaration and Release form (FF-104-FY-21-128) is not in your file. This form must be completed and signed by an adult member of your household. The form authorizes us to collect information which is used to determine your eligibility to receive FEMA assistance. All applicants who apply for assistance must sign the Declaration and Release, FEMA Form FF-104-FY-21-128, and provide a form of identification prior to the receipt of disaster assistance.
