<p><strong>Lease</strong> (housing agreement)</p> <p>Make sure your document includes:</p> <ul> <li>Your name or your co-applicant's name.</li> <li>Your home address listed on your application.</li> <li>Proof that it was dated within a year before the disaster began. Or</li> <li>If the document is dated after the disaster, it must clearly state that you or the co-applicant lived in the damaged home at the time of the disaster.</li> <li>The name and contact information of the person or organization providing verification.</li> </ul> <p><b>Note:</b> Please make sure your lease clearly shows it was in effect at the time of the disaster. If possible, send a copy with the landlord's signature. If their signature is not available, you can write a note explaining that you don't have a signed copy.&nbsp;</p>
