To be considered for FEMA assistance, you must provide a copy of your insurance policy verifying your name and damage dwelling address, the name of the insurance company, phone number, policy number, policy dates and type of coverage. In addition, provide a breakdown of whether you have real property, personal property and/or miscellaneous coverage and loss of use/ALE (Additional Living Expenses).
