Program Description

A Presidential Memorial Certificate (PMC) is an engraved paper certificate, signed by the current President, to honor the memory of honorably discharged Veterans.

General Program Requirements

Eligible recipients, including the Veteran's next of kin and loved ones, or someone acting on their behalf, may apply for a PMC. You may request more than one certificate.

You may not request a PMC for a living Veteran.

A Veteran must have an honorable discharge or a general discharge under honorable conditions. Certain National Guard and Reserve members may also qualify. Commissioned Officers of the Public Health Service (PHS) and National Oceanic and Atmospheric Administration (NOAA) qualify as active duty members and Veterans once discharged.

Application Process

To make a request, eligible recipients, or someone acting on their behalf, must complete the Presidential Memorial Certificate Request Form (VA Form 40-0247, PDF, 721 KB). You can submit the form using one of the methods below:

We do not accept requests via email.

You must include a copy of both the Veteran’s discharge and death certificates. We can’t process a request without proof of honorable military service. Please send copies only. We will not return original documents.
